Our team will guide you through the online intake forms and check-in and will connect with you after your video (or in-person) meeting to continue the conversation and coordinate the next steps.
We utilize a completely HIPAA-compliant telemedicine service (Aestheticrecord.com) that is the safest and most secure method to conduct your virtual consultation. Forms to be filled out and signed before your appointment, either via the Patient Portal or download the below PDFs, sign and email them to spa@skintologyapothecary.com.
